Service Request Form

This form is to be used by the public as an easy way of submitting Sanitation Service Requests electronically. Use this form to make request regarding picking up bulky items, Metal Household Appliance Items, Brush Items, Move In / Move Out Pick Up, and/or Services Not Complete. In addition, you may also use this form to request for replacing Black/Blue/Green broken/missing/damaged containers. Requests are processed in 2-3 business days. To better serve your needs, please complete steps 1 through 6 below as required:
